چکیده

We read with great interest the article by Kido et al 1 in an issue of CHEST (January 2012) demonstrating mucosa-associated lymphoid tissue (MALT) lymphoma translocation gene 1 rearrangements in BAL fl uid (BALF) in four of fi ve cases of pulmonary MALT lymphoma. The use of BALF is debated, but the authors elegantly demonstrate that BALF may be useful for the diagnosis of pulmonary disease, particularly when lymphoma is suspected. The authors report an elevated number of lymphocytes, median 30%, in BALF from patients with pulmonary MALT lymphoma, which did not differ from control samples (23.6%). However, the authors did not report the phenotype of the lymphocytes or the results of clonality analysis. Indeed, we previously reported, for a series of 44 patients, an increased proportion of lymphocytes in BALF (31.5%) and a median B-lymphocyte proportion (CD19 or CD20) of total alveolar lymphocytes of 20% (normally , 10%); moreover, clonality analysis of alveolar B cells showed a B-cell clone in 82% of the cases. 2 In a prospective study, specifi city and sensitivity of BALF clonality results were reported to be 97% and 95%, respectively. 3

The Value of LDH Level of BAL Fluid in Differentiating Benign from Malignant Solitary Pulmonary Nodules

Background: Serum lactate dehydrogenase (LDH) concentration is an indicator for tissue injury. It may be secreted locally in many conditions. For the first time, this study was performed to investigate the value of LDH level in bronchoalveolar lavage fluid (BALF) in differentiation of benign from malignant single pulmonary nodules (SPNs) and to assess its relationship with serum LDH levels. Met...

Breast MALT lymphomas: a clinicopathological and cytogenetic study of 9 cases.

Primary breast mucosa-associated lymphoid tissue (MALT) lymphomas are uncommon and restricted diagnostic criteria should be used to exclude breast involvement by systemic lymphomas. The molecular pathogenesis of primary breast MALT lymphomas is not clear because of the rarity of the disease. Generally the molecular studies of MALT lymphoma in extr...

Level of Nitric Oxide in Bronchoalveolar Lavage Fluid of Asthmatic Mice Model

Background: Asthma is a chronic inflammatory disease with multifactorial and complicated mechanisms. Elevated level of exhaled Nitric Oxide (NO) in asthma and other inflammatory lung diseases has led to many studies examining NO as a potential marker of airway inflammation. Objective: This study was designed to determine the level of NO in Bronchoalveolar Lavage (BAL) fluid during early and l...

Detection of galactomannan in bronchoalveolar lavage of the intensive care unit patients at risk for invasive aspergillosis

Background and Purpose: Invasive aspergillosis (IA) is one of the most common life-threatening fungal infections among the critically ill patients including intensive care unit (ICU) patients. Delayed diagnosis and therapy may lead to poor outcomes. Diagnosis may be facilitated by a test for molecular biomarkers, i.e. detection of galactomannan (GM) antigen based on enzyme immunoassay, which is...

MALT lymphoma meets stem cells

Mucosa-associated lymphoid tissue (MALT) lymphomas are a distinct clinico-pathologic entity mainly associated with chromosomal translocations involving the MALT1 gene. Therefore, these chromosomal rearrangements have been traditionally used to identify tumor MALT lymphoma B-cells, and they have always been detected in differentiated tumoral B cells. However, the hematopoietic progenitor and ste...
